This notice is generated when an insurance company notifies the SOUTH CAROLINA DEPARTMENT
OF MOTOR VEHICLES that an insurance policy has been cancelled. If the SOUTH CAROLINA
DEPARTMENT OF MOTOR VEHICLES cannot verify that a new policy has been secured this
notice is sent to the registered owner of the vehicles associated with the cancelled
policy. Once the notice has been received you must verify that you have insurance
or have met a non-insurance requirement and therefore, insurance is not required.
This insurance information can be verified electronically using the SC ALIR system.
This notice is generated when the SOUTH CAROLINA DEPARTMENT OF MOTOR VEHICLES could
not verify the insurance information you provided following registration. If the
SOUTH CAROLINA DEPARTMENT OF MOTOR VEHICLES could not verify your insurance this
notice is sent to the registered owner of the newly registered vehicle. Once the
notice has been received you must verify that you have insurance or have met a non-insurance
requirement and therefore, insurance is not required. This insurance information
can be verified electronically using the SC ALIR system.
The green FR10 forms are issued by law enforcement officers following a reportable
accident. Each party with a motor vehicle involved in the accident is provided the
FR10 form in order to verify liability insurance at the time of the accident. This
insurance information can be verified electronically using the SC ALIR system. If
you have received this form and did not have insurance at the time of accident you
should indicate that liability insurance was not in place at the time of accident
in the bottom, left-hand corner of the FR10 form.
An FR 5 Accident Suspension Notice is generated when the SOUTH CAROLINA DEPARTMENT
OF MOTOR VEHICLES has failed to receive an FR10 transaction which verifies your
liability insurance at the time of accident.
If you need to verify your insurance coverage you should contact your insurance
company or agent. If you have received an FR4 Cancellation, FR31 Verification Request,
or an FR5 Accident Suspension notice you must provide your agent or insurance company
representative with the Reference Number which is located at the top right-hand
corner of the suspension notice. If the reference number is not available, a VIN
search can be done by your agent or insurance company representative. If you have
received a green FR10 Form you must provide your agent or insurance company representative
with the FR10 Audit number associated with your accident party. Using the Reference
Number or FR10 Audit number and the SC ALIR system your agent or insurance company
can verify your insurance information with the SOUTH CAROLINA DEPARTMENT OF MOTOR
VEHICLES in real-time.
You should inform your insurance company or agent that all South Carolina licensed

You are required to surrender your South Carolina automobile license plate or obtain

and mail this to the SOUTH CAROLINA DEPARTMENT OF MOTOR VEHICLES at the address
